As per the OP, I have witnessed it some in over 36 years of gaming...

I despise those losers...  They are insignificant people.   If I caught them, I embarrassed them at the table and kick them out of the game to be banished forever...

If I see them at gaming conventions in scoring events, I will ensure the DM's/GM's know about them as well with proof.

This is one of the things that the HackMaster 4th Edition HSST(HackMaster Smartass Smackdown Table) is for. Or for extreme situations there's the v2.2 of it.

The beauty of it is that these tables can be used for any gamesystem, not just HM...

I've witnessed the same effect (cheaters never prosper.) Overconfidence + ignorance = comedy of error.
 
Stuff like cheating by rolling high when you're supposed to roll low, putting portable holes into bags of holding, trying to leap over a 40' chasm with a confirmed natural twenty.
