Here is my take on Fear (called Ambient Fear) and Terror. This is version 1 (rough draft).

Unlike your Injury option, which could come into play in any combat or just by falling, this one is specific to creatures or supernatural effects (locations, items, spells, etc, I guess?) so it's only in play if the DM includes it. While it's good to have a common rule that can be reused, I can also see having creatures projecting fear that works in a different way. So it's good if that's what the DM wants the monster/area to have, and not if it's not.
One suggestion for an addition: you might require that the source of Terror also generate an area of Ambient Fear to give some warning and to build up the suspense. Terror seems pretty nasty to come out of nowhere.

Like the enigmatic aura inhabiting the towns in 'Uzumaki' or Stephen King's 'It' (also, cities in war zones)... various sorts of projected mania. What happens if you leave the area? Do all the effects immediately fall off?
There's a cursed stone in the outdoor portion of the new Death Frost Doom that creates an area of fear like that... we jumped in and out and figured out it's general proportions and marked it.

That seems like it would make sense for some creatures, as a supernatural armor or somesuch... but there are those other times where sudden revelation comes out of nowhere and some folks flee or crumble.
Even for purely natural creatures/events.
Seems like sudden death should be VERY unlikely... vs. involuntary urge to flee (or blindly charge in attack).
rawma, I like that suggestion. For some creatures (say like Dracula) it would make sense to tie them all together.
There was a little confusion at the table yesterday about the "present" and "ambient" fear, so I'm splitting them up and making the rules clearer.
I like options, so I think that in some cases one would want ambient fear to foretell Terror. In others, the Dark God Thing rises from the depths and everyone on the boat either drops dead, goes comatose, or panics madly until they find some respite. :-)
I will update this thread when version 2 is ready.
