#1 For game-useability rather than prettiness, I find the BECMI Gazetteer 8-mile hex maps are tops.  My Northern Reaches map especially got used ragged.

#2 For prettiness and useability combined, 1983 World of Greyhawk box.  Only the too-large 30-mile scale keeps this from being #1.
