What's Really Important to Have in a Medieval Authentic RPG?

It's a balancing act. The point is that 'authentic' is not the same as 'realistic'. "Authentic" in RPG design has to be emulative; it needs to be that way to be gameable.

It's not a perfect analogy by a long shot but the most important thing is to get rid of that kind of thinking, make players think of the nobility like old school mafia families. I think if anything the bigger issue is looking at medieval nobility through a lens of quasi-marxist history; assuming they were all uncaring bastards who lived for decadence and treated all their vassals as mere tools and their peasants as expendable garbage.  Of course, there were some people like that, but they were the exception, not the rule.

Which begs the sincere question:  Is there any other option to allow for some sort of freewheeling adventure?

There are lots of options, but the smart choice is to work for a lord, or the church, or a guild, or some other source of influence.

The setup of my "Inquisitors Campaign" structure in Cults of Chaos provides one example of how to do this.

What doesn't fit in a Medieval Authentic campaign, at least for anything other than a group of brigands, is the typical D&D party of "freewheeling independent operators doing whatever they want and answering to no one".

Hear, hear.

And as Sir John Hawkwood and the White Company showed, the difference between "brigands" and "mercenaries" was often "when is the last time the lads had a good meal?"

Never really had an issue with that, the only time my players went out of their way to attack the nobility it was simply because the nobles were the ones with the money. Have had more issues with players disrespecting noble allies and then being shocked when the nobles just didn't laugh it off. Think they'd run through too many modules in which NPCs just grin and bear being shat on because it's that NPC's job to help the PCs along with their quest.

NPC as vending machine; either insert thing of value to receive help with quest, or insert sword to receive XP.
