I have never played in a game with level titles and to be frank find it a bit odd that someone should begin being adressed "Duke" or "Count" or whatever just because they hit Level 8 or whatever.

That said, however, I do think D&D-esque games with levels should have a descriptor attached to levels (or level intervals) to players and the GM an idea of exactly how much experience (as in real-life, not numerical XP) the average NPC of that level has.

You know, terms like Off-the-farm, initiate, apprentice, professional, veteran, hardened veteran, master, Best-in-the-land, etc, that do not necessarily imply nobility status.
